As a courtesy, we will attempt to verify your insurance coverage, notify you of any required deposits and submit bills directly to your insurer. However, it is your ultimate responsibility to obtain pre-authorization for medical procedures if required by your insurance.
Please understand that if you did not supply adequate insurance information at or before time of service, then your case may result in a denial due to claim filing limitations and pre-certification requirements.
You will receive a bill from Ochsner Lafayette General for your medical procedure/hospital stay that covers diagnostics, equipment, supplies, drugs and nursing care. This bill will not include services provided by a doctor.
In addition, the ER physician, your primary physician, surgeon, anesthesiologist, radiologist, pulmonologist and pathologist may bill you separately for their professional services.
Payment is expected from you and your insurance carrier with 45 days of the claim submission date. Statements are not sent to you until your insurance carrier(s) has completed processing your claim(s). Should your insurance carrier delay processing of your claim, you will be billed for services rendered.

If you were a patient at Ochsner Abrom Kaplan Memorial Hospital, Ochsner University Hospital & Clinics, Ochsner Lafayette General Endoscopy Center, Ochsner Lafayette General Urgent Care Center (formerly Family Health Plaza) or any of our affiliate hospitals, then you will receive a bill directly from that facility.
As of Oct. 1, 2015, the Emergency Department providers (doctors, nurse practitioners, physician assistants with Schumacher Group) that work at Ochsner Lafayette General Medical Center, Ochsner University Hospital & Clinics, Ochsner St. Martin Hospital, Ochsner Acadia General Hospital, Ochsner Abrom Kaplan Memorial and Ochsner Lafayette General Orthopedic Hospital—An Ochsner Lafayette General Medical Center Campus are now in-network with Blue Cross Blue Shield of Louisiana.
